Customer Experience:
To register, the recipient of this certificate may either
(i) go to the web-site www.lvigolfnbeauty.com and follow the registration guidelines, or
(ii) complete the requested registration information and return toLifestyle Vacation Incentives, LLC.
Customer may review the list of participating salons and golf courses
with available services on www.lvigolfnbeauty.com.

The customer then e-mails their desired golf course or beauty salon location, appointment date
and time requested from the website or calls our toll free number at least three weeks in advance
of desired appointment. LVI will confirm your reservation location, date and time by e-mail.
